Đau bụng 3 ngày, nam bệnh nhân 42 tuổi, ở Thái Bình chủ quan cứ nghĩ mình bị dạ dày, đi khám mới biết bị bệnh lý hiếm gặp ở thận và có nguy cơ gây hỏng thận nếu không được chẩn đoán và điều trị kịp thời.
Theo BS Nguyễn Xuân Tuyến, chuyên gia về thận, đau bụng là triệu chứng phổ biến mà nhiều người gặp phải, cũng như hay gặp trong nhiều bệnh lý. Đó có thể là những bệnh lý đơn giản như rối loạn tiêu hoá, viêm dạ dày... hay đến cả những bệnh lý cấp cứu nguy hiểm khác như tắc ruột, thủng ruột, viêm ruột thừa. Đặc biệt, nhiều bệnh lý hiếm gặp cũng có triệu chứng này.

Bệnh nhân nam, 42 tuổi đến khám do bị đau bụng âm ỉ 3 ngày nay, thăm khám không thấy dấu hiệu gì đặc biệt ngoài dấu hiệu nhiễm trùng được biểu hiện qua xét nghiệm máu, kể cả chụp X-quang và siêu âm không ghi nhận bất thường để giải thích tình trạng đau bụng của người bệnh.
Không bằng lòng với kết quả, BS Tuyến chỉ định thêm chụp cắt lớp vi tính ổ bụng có tiêm thuốc cản quang, nhằm xác định rõ nguyên nhân gây đau bụng.
Hình ảnh chụp cắt lớp vi tính (CT-Scanner) ổ bụng cho thấy hình ảnh nhồi máu thận phải do tắc một nhánh của động mạch thận. Ngay sau đó bệnh nhân được nhập viện điều trị và chỉ sau 7 ngày, tình trạng sức khỏe của bệnh ổn định và hết đau bụng.
Nhồi máu thận là gì?
Nhồi máu thận là tình trạng thiếu máu nhu mô thận do sự tắc nghẽn hoàn toàn hoặc một phần dòng máu đến thận. Nhồi máu thận thường xảy ra một bên, do nguyên nhân tắc động mạch thận.
Nguyên nhân gây tắc nghẽn mạch thận có thể do:
- Huyết khối thường bắt nguồn từ tim hoặc động mạch chủ: Có thể gây tắc hoàn toàn động mạch thận chính, hoặc các động mạch nhánh nhỏ hơn.
- Các mảng xơ vữa: Thường dẫn đến teo thiếu máu cục bộ thứ phát hơn là nhồi máu thận.
- Tổn thương mạch thận: Lóc tách thành động mạch.
Dấu hiệu của nhồi máu thận có thể gặp như:
- Đau bụng: vị trí mạn sườn, thắt lưng;
- Buồn nôn, nôn;
- Sốt.
BS Tuyến tiếp nhận bệnh nhân đến khám trong tình trạng đau bụng âm ỉ. Nếu không có nhạy cảm lâm sàng để chỉ định chụp cắt lớp vi tính, bác sĩ rất dễ bỏ sót căn bệnh này.
Ở bệnh nhân này đã được nhận định đúng đắn và khẳng định chính xác bằng kết quả chuẩn đoán hình ảnh công nghệ cao, giúp bệnh nhân kịp thời điều trị, thoát khỏi nguy hiểm. Vì nếu chẩn đoán bỏ sót hoặc chậm trễ trong quá trình điều trị, chắc chắn sẽ gây giảm hoặc mất chức năng thận.
BS lưu ý, người dân khi cần đến các cơ sở y tế khám ngay khi phát hiện cơ thể có dấu hiệu bất thường như mệt mỏi, chán ăn, gầy sụt cân, mất ngủ, đau bụng kéo dài... tránh bỏ sót bệnh và nhằm được chữa trị kịp thời nếu không may phát hiện mắc bệnh lý.
